The Occupational Safety and Health Administration has issued a $11,675 fine against Hayward Baker for three federal violations that lead to a worker’s death.

Last August, a 39 year-old construction worker from Minneapolis was killed after his fall prevention cord became tangled in a drill. The man was dead before other workers could reach the emergency shut-off button. All three violations are related to the improper set up of the shut off-button and anchor point.

Workers were drilling under an apartment complex to improve the bank of a creek and to improve the safety of the residents. Hayward Baker has since changed its manual safety procedures.

Foods containing high amounts of organic brown rice syrup have been found to contain dangerously high levels of arsenic, according to a study out of Dartmouth College.

Researchers found that organic baby formulas, cereal bars, and energy shots with brown rice syrup have arsenic levels higher than the EPA allows in drinking water. Brown rice syrup is used as a sweetener and as a healthier alternative to high fructose corn syrup.

Those involved with the study encourage parents of infants to not use baby formula with brown rice syrup, but that adults who occasionally eat food products with brown rice syrup will probably be okay.

A fire that broke out in a Northfield assisted living facility was deemed arson after a 46 year-old resident admitted to starting the fire.

Nearly 40 residents were evacuated after someone found saw a fire in an unoccupied room. While the fire was put out quickly, smoke had spread throughout the facility. One resident was taken to the the hospital for observation and other was admitted after complaining of chest pains.

Officials say the arsonist told authorities he started the fire by igniting paper over an open flame but did not give a reason why. Some of the evacuated residents were accommodated by the Northfield Hospital while repairs are being made to the facility.

A massive car pileup in Gainesville, Florida over the weekend sent 18 people to the hospital and claimed the lives of 10 people.

The accident happened around 3 am Sunday morning on Interstate 75. Dense fog in the area and smoke from nearby brush fires lowered visibility significantly. Florida Highway Patrol shut down a stretch of the interstate due to the dangerously low levels of visibility for several hours.

Less than a quarter hour after reopening the highway, at least a dozen cars, six tractor-trailers, and a motor home collided. Witnesses recall seeing cars struck multiple times, cars wedged under the tractor-trailers, and cars bursting into flames. It was so difficult to see that rescue crews could only find victims by following the sounds of their screams.

Our deepest sympathies are with the family and friends of those affected by the accident.

The search for 16 passengers, including two Americans, on board a cruise liner continues just days after it crashed into the rocky ocean floor near Italy’s Giglio Island.

About 4,200 passengers and crew members were on the Costa Concordia when it sunk on its side Friday night. Reports state serious errors on the captain’s part is to blame for the wreck that has claimed at least six lives. The ship traveled too close to shore and rocks ruptured the ships sides.

Dozens of passengers were injured as they scrambled to get aboard the limited number of lifeboats. Some jumped into the icy sea and swam to shore while others waited for hours for help to arrive. The ship’s captain has been arrested for manslaughter, causing the ship wreck, and abandoning the ship before all people on board were evacuated.

A San Diego Superior Court jury awarded a man $7.5 million for a slip and fall accident that occurred at Starbucks.

The man was walking from the cashier to the pickup counter at a Starbucks in 2008 when he slipped on the floor the manager was mopping. He injured his head and has not been able to return to work as a chiropractor since the fall.
